Contest Rules and Regulations

GiveMeLASIK.com is owned and operated by Kleiman | Evangelista Eye Center. Serving Dallas, Fort Worth and all of Texas since 1983, Kleiman | Evangelista Eye Center, one of the first to perform laser eye surgery in the DFW metroplex, offers Blade-Free HD IntraLASIK (all-laser LASIK) surgery– one of the safest, most trusted procedures available today.

Official Contest Rules (subject to change without notification):

Kleiman | Evangelista Eye Center and its affiliates reserve the right to extend the final submission date of the contest, as well as (but not limited to) general and fundamental changes and amendments to the contest rules in complete or partial form.

Participants must be over the age of 18, legal U.S. citizens (and able to provide proof of U.S. citizenship if requested) and determined a candidate for the IntraLASIK procedure by Kleiman | Evangelista Eye Center. If you are submitting a video for a friend or family member (anyone other than yourself), you must be over the age of 18 to submit a video for them. Further, contestants must be U.S. citizens living in the Dallas/Fort Worth Metroplex or surrounding areas of Texas to be eligible for the contest.

The Kleiman | Evangelista Eye Center Win Free HD IntraLASIK Contest will feature a selection of judges to choose their favorite videos from the submissions, which will then be considered "Finalists". Finalist submissions will be displayed on the GiveMeLASIK.com website where voting (up to once per day, per household / IP address) is open to the public. Two videos submitted by contestants will be chosen as winners for the TexasLasikContest.com contest to receive free Blade-Free HD IntraLASIK from Kleiman | Evangelista Eye Center. Winners must be eligible for LASIK and IntraLASIK in order to accept the prize, and Kleiman | Evangelista Eye Center reserves the right to refuse any treatment to the winner if they do not qualify.

The grand prize voting session of the Kleiman | Evangelista Eye Center Blade-Free IntraLASIK Contest by the public will count as a percentage of the final vote for choosing the grand prize winners. Ultimately, Kleiman | Evangelista Eye Center reserves the right to make the final selection of the two Grand Prize winners during the public voting process.

The grand prize of free bilateral Blade-Free HD IntraLASIK, depending on qualification for the procedure by the winner, will result in free pre-operative tests, the actual HD IntraLASIK procedure on BOTH eyes, and any necessary post-operative follow-ups / check-ups following the procedure (this includes typical and customary LASIK follow-ups for up to one (1) year after the procedure is performed).

Contestants MUST submit video content that is appropriate for family viewing, and may not contain profanity, nudity, references to illegal drugs/activity, destruction/defacement of private or public property, or any other depiction that may be interpreted as an offensive act or exhibition. Kleiman | Evangelista Eye Center reserves the right to disqualify and remove submissions that are inappropriate in nature. Additionally, video and audio content used in the contestant submissions MUST NOT contain copyrighted material, or material that they do not have permission to use.

In an effort to keep video submissions clear and understandable to as many people possible in the DFW area, we ask and require that all submissions be produced (or subtitled) in the English language. Kleiman | Evangelista Eye Center reserves the right to deny foreign-language video entries approval for submission.

By submitting video entries, contestants relinquish ownership rights of all content featured in the video. Kleiman | Evangelista Eye Center will consider all material submitted as property of Kleiman | Evangelista Eye Center, and may use these videos in any way that it chooses.

Contestants may enter only ONE video to qualify, and may not enter multiple videos. Contestants MAY enter a video for another person (such as a friend or family member), but may not enter a video for both themselves and another person separately. Similarly, contestants may only enter a video for a SINGLE contestant (not as a multi-contestant submission or similar). Kleiman | Evangelista Eye Center reserves the right to determine the kind of submission violation that any video may exhibit or pose.

The grand prize of free HD IntraLASIK at Kleiman | Evangelista Eye Center is not valid on (or in conjunction with) any other procedure, treatment or vision correction outside of the LASIK procedure. This includes the exclusion of ICL, IOL and Cataract procedures that are also available at Kleiman | Evangelista Eye Center.

The Kleiman | Evangelista Eye Center HD IntraLASIK Contest expires on December 31, 2009, at 12:00pm (Noon) and all contestants must enter their videos on or before that date to be eligible for the Grand Prize of FREE bilateral Blade-Free HD IntraLASIK from Kleiman | Evangelista Eye Center.

Registration Terms and Conditions:

By registering with GiveMeLASIK.com, contestants agree to receiving promotional material from Kleiman | Evangelista Eye Center and its related affiliates. Kleiman | Evangelista Eye Center will not sell, lease or distribute your contact information to any outside parties unaffiliated with Kleiman | Evangelista Eye Center, and will use your contact information for the sole-purpose of offering special promotions and incentives from Kleiman | Evangelista Eye Center, as well as important contestant notifications from the Kleiman | Evangelista Eye Center HD IntraLASIK Contest.

Registrants have an opportunity with each email to unsubscribe from our contact listings if they decide to discontinue contact from Kleiman | Evangelista Eye Center and it's affiliated partners in vision correction.

$1500-Off Coupon

All contestants who correctly submit a video will be eligible to receive a special $1500 coupon to use toward their Blade-Free HD IntraLASIK procedure at Kleiman | Evangelista Eye Center. This coupon can only be redeemed at Kleiman | Evangelista Eye Center in Arlington, Texas, and cannot be combined with any other offer or promotion. It is good for use towards one (1) bilateral (both eyes) HD IntraLASIK procedure. Contestants who qualify will receive the coupon after the first phase of the contest is complete (after December 31, 2009) and may use the coupon at any time before February 28, 2010.

The Grand Prize winner may contribute their $1500-off coupon toward someone else's procedure (friend, family member, coworker, etc.) but any recipient may not use the coupon in combination with their own coupon, an additional promotion or special.

Kleiman | Evangelista Eye Center and the contest staff reserve the right to refuse delivery of the $1500-off coupon to anyone at any time without notice. This provision is set in place in the case that some contestants may claim to "submit" their video but their video has been deemed "inappropriate", incorrectly submitted, submitted past the contest submission deadline (December 31, 2009, Noon CST) or any other reason cited or un-cited by the contest staff.

Finalist Voting Terms and Conditions:

The Kleiman | Evangelista Eye Center Win Free HD IntraLASIK Contest requires users to submit only one vote per day, per IP address. This is in an effort to curb abuse of the voting system and any other malicious activity in attempt to tamper with voting results. Kleiman | Evangelista Eye Center reserves the right and ability to disqualify any video finalist who is suspected of deliberately attempting to gain an unfair advantage over other contestants, including (but not limited to) inflating their own voting results themselves or through a 3rd party (friend, family member, etc.). This is especially the case with videos that have been voted on from the same IP address a large number of times, but smaller inflation attempts are also subject to review.

If there are any questions or concerns about the Kleiman | Evangelista Eye Center Win Free HD IntraLASIK Contest or its registration terms and conditions that are unclear, call (800)714-2020. Kleiman | Evangelista Eye Center will not be responsible for clarification problems if any dispute is not completely resolved by the involved participant or parties if they in fact do not clarify with a Kleiman | Evangelista Eye Center representative.
